Notary Depot
My main project company, Notary Depot, was redone in Ruby on Rails back in early 2005. It now serves the needs daily of thousands of Notaries Public nationwide. Being that it is the project that I first used Rails for, it has seen tremendous improvements over the course of the last two years. In its current state everything has been tidied up, locked down and re-factored to the point where I am very satisfied with the code-base. It is managed via trac with subversion integration and deployed with Capistrano to a dedicated server running Litespeed.
